Membership Forms

• AAA Auto Pay Plan for Membership–Credit or Debit Cards: Complete this form to set-up automatic payment of annual membership renewal dues using a credit or debit card.
• AAA Auto Pay Plan for Insurance and Membership–Credit or Debit Cards: Complete this form to set-up automatic payment of annual membership renewal dues and insurance policy installments using a credit or debit card.
• AAA Auto Pay Plan for Membership and/or Insurance–Checking Account: Complete this form to set-up automatic payment of annual membership renewal dues and/or insurance policy installments using a checking account.
• AAA Auto Pay Plan Revocation Request–Termination of Automatic Payment Withdrawals from Checking Account: Complete this form to terminate enrollment in the AAA Auto Pay Plan for membership and/or insurance policies.
• Emergency Road Service Report: Complete this form to report a compliment, complaint, damage/faulty repair, or overcharge related to an emergency road service call.
• Roadside Assistance Reimbursement Application: Complete this form to apply for reimbursement of an emergency road service call or locksmith service for which you paid out of pocket.
